TRANSFER OF CADAVER

About the service

The Municipal Health Office issues permit for transfer of cadaver upon presentation of death certificate, as to the cause of death (communicable or non-communicable disease) and after inspection of the cadaver. Provided, that the procedure must be done in the most sanitary manner without detriment to the health of the people.

How to avail of the service

1. REQUEST

Client requests for a transfer of cadaver to other towns. Client presents a copy of death

certificate of cadaver to be transferred to Sanitary Inspector

2. ASSESSMENT

Interview the client. Assess the cadaver on the cause of death. Advise the client to go the Municipal Health Officer for further interview and assessment

3.PAYMENT OF FEES

Advise the Client to pay the fee

4. ISUANCE OF PERMIT

Client presents Official receipt to Sanitary Inspector. Prepare the permit for the transfer of cadaver/ sign and forward to

MHO for signature. Record the name of client in the logbook. Release the permit

SECURING SANITARY PERMIT

About the service

The Municipal Health Officer issues a sanitary permit to operate in all business establishments after the actual inspection. Health cards are being issued to operators after laboratory and physical examination is done.

How to avail of the service

1. Request

Client requests for issuance of sanitary permit. Interview the client

2. Assessment

Client submit specimen for laboratory examination. Client is advised to get the result of the laboratory examination after 24 hours.

3. Payment of fees

Advise the Client to pay the prescribed fees

4. Issuance of health certificate

Client gets laboratory result and present official receipt for payment of fees. Medical Technologist issues health certificate.

5. Physical examination

Client is advise to go the MHO for physical examination bringing with him/her the result of the laboratory examination

6. Issuance of sanitary permit

Client presents Health Certificate Sanitary Inspector prepares the Sanitary Permit/sign and forward to MHO for signature. Record the name of the client in the logbook. Release Sanitary permit

REQUESTING FOR ISSUANCE OF MEDICAL CERTIFICATE

About the service

Firms and government agencies may require Health certificates from certain persons. This is especially true for those people seeking employment. Schools also require students to secure a medical certificate before they are allowed to enroll.

About the service

Treatment of minor surgical cases can be attended to by the Municipal Health Center during its clinic hours from 8:00 to 12:00 noon and from 1:00 to 5:00 in the afternoon.

AVAILING OF MEDICAL CONSULTATION

About the service

The purpose of this service is to diagnose and treat illnesses and give appropriate medical services. A service is available at the Municipal Health Office (MHO) to any person/individual who needs medical assistance.

AVAILING OF IMMUNIZATION SERVICES

About the service

The purpose of this service is to immunize children 0-11 months old from 6 immunizable diseases. The Municipal Health Office (MHO) also immunizes pregnant mothers to prevent the occurrence of Tetanus Neonatorum in infants.

This service is provided free of charge.

1. Registration

Client gets priority number at the waiting area. BHW, MHM on duty take vital signs

Midwife asks data of child to be immunized and looks at past immunizations given to the child.

2. Immunization

Midwife on duty gives immunization as requested

3. Post Immunization Instruction

Midwife gives mother post immunization instructions and informs her about the schedule for the next round of immunization

AVAILING OF FREE ANTI-TUBERCULOSIS DRUGS

About the service

The Municipal Health Office (MHO) manages an anti-tuberculosis program. The purpose is to identify and treat patients with tuberculosis (TB). Drugs and medicines are provided free of charge.

Who may avail of the service

Any person, 10 years old and above, who displays the following symptoms may have tuberculosis:

* Persistent coughing for 2 weeks or more

* Fever

* Progressive weight loss

* Chest or back pains

* Hemoptysis or recurrent blood streak sputum

* Loss of appetite

* Tiredness/night sweating

How to avail of the service

1. Inquiry

Client gets priority number at the waiting area

2. Client receive instructions

MHM, MHN on duty interviews the client. Client receives instructions for proper sputum collection

3. Collection and submission of specimen

Midwife collects sputum specimen and submits it to the Medical Technologist for examination. Client receives information as to the date of release of result.

5. Enrollment of patient

TB nurse coordinator or Midwife on duty:

> Assesses the patient if illegible as National

Tuberculosis Program (NTP) beneficiary

> If eligible, enrolls patient and issues NTP identification card

> Gives patient information education about TB Disease and Control the importance of DOTS (Directly Observed Treatment Short Course)

> Starts issues initial TB drugs supply to patient for 1 week and instruct to come back to the RHU every week.

> Midwife instructs treatment partner and patient where to report for his daily intake of TB drugs and schedule of sputum follow-ups.

AVAILING OF DENTAL EXAMINATION/TOOTH EXTRACTION SERVICES

About the service

This service is available to pre-school and school age children, pregnant mothers and other adults to prevent and treat dental diseases. Clients may avail of the service at the Municipal Health Office. Tooth extraction and dental consultation is available every Monday only.

AVAILING OF BIRTHING FACILITIES

About the service

One of the programs of the Department of Health (DOH) is that each LGU if possible should establish a birthing clinic in order to provide an environment conducive to the best care for mother, infant and to eliminate maternal and infant mortality rate.

2. Monitoring stages of labor

First stage of labor (starts when musculature of the uterus starts true pronounced constructions and brings about progressive cervical effacement and dilatation of the cervix)

Second stage of labor starts with complete dilatation and effacement of the cervix and end with the actual birth of the infant. Patient is placed in the RHU delivery Area

Third stage of Labor (Expulsion of Placenta)

> Forth stage of labor (Recovery Period) still in the delivery room
